What is Enhanced Nursing Home Transition?

It is the process of assisting people to move from a nursing home to the community if that is what he or she would like to do. It begins with identifying people who wish to move out of a nursing home and supporting that person with coordination of the activities related to moving into the community. Enhanced Nursing Home Transition may include assistance in locating housing, establishing a household, and setting up services necessary for a person to live independently. All of these tasks are accomplished through extensive consumer involvement.
